First priority when several problems compete
ABC
  • AAirway: obstruction, stridor, swelling, secretions
  • BBreathing: rate, SpO2, accessory muscles, breath sounds
  • CCirculation: bleeding, BP, pulses, perfusion
Pearl: A beats B beats C, except cardiac arrest: CAB (compressions first).
When you need it: Four clients need you at the same moment. Which threat do you treat first?

The nursing process, in order
ADPIE
  • AAssess: collect data first
  • DDiagnose: name the actual or potential problem
  • PPlan: set goals and expected outcomes
  • IImplement: carry out the plan
  • EEvaluate: did it work? Revise if not
Pearl: Assess before acting, unless the client is in arrest, anaphylaxis, or hemorrhage.
When you need it: The stem asks what the nurse should do first and the data is incomplete.

  • PPhysiological: airway, breathing, circulation, fluids, pain
  • SSafety: falls, infection, medication safety
  • LLove and belonging: family, support, communication
  • EEsteem: autonomy, dignity, privacy
  • SSelf-actualization: growth, wellness goals, teaching
Pearl: Lower levels win. Pain is physiological. Teaching is self-actualization.
When you need it: No ABC problem is present. Two needs compete: one is physical, one is emotional.

What makes one client the priority over another
New, Worse, Unexpected
  • NewNew onset beats a chronic, known problem
  • WorseA worsening trend beats a stable or improving one
  • UnexpectedA finding unexpected for the diagnosis beats an expected one
Pearl: Trends matter more than single values.
When you need it: Two clients have serious diagnoses. One is stable at baseline, one just changed.
